Change of Major Form

Students must submit this form with the proper signatures when required to change their Program/Plan (Change of Major).
Required Signatures: Student, Department Chair


Change of Minor Form
Students must submit this form at least one semester before graduation.
Required Signatures: Student, Department Chair


Internship Enrollment Form
Students must submit this form at the time of registration. A copy of the Internship proposal must accompany this form.
Required Signatures: Student, Faculty Supervisor, Department Chair, Assistant Dean


Independent Study Form
Students must submit this form at the time of registration. A copy of the Independent Study or Mentored Research proposal must accompany this form.
Required Signatures: Student, Faculty Supervisor, Department Chair, Assistant Dean


Interdisciplinary Concentration Application Form
Students must submit this form with the proper signatures in order to declare and interdisciplinary liberal learning concentration.
Signatures Required: Student, Concentration Advisor, Major Advisor
